How do I obtain a copy of a Death record?

Note: The Town of Babylon Clerk’s Office will have a copy of the Death record if the person was pronounced deceased in the Town of Babylon. If the person was pronounced deceased in a hospital, please call the Town in which the hospital resides. See attached list of Towns and Villages.

In order to obtain a certified copy of a Death Certificate by mail, you will need to complete and mail:

  1. The attached Death Certificate Request form; (Death Request application form) send proof of relationship to the deceased which be either a copy of your birth certificate or copy of your marriage certificate. If neither of these are not an option please call 631-957-3007 for additional instruction
  2. Copy of your driver’s license
  3. Money order made out to the Town of Babylon in the amount of $10.00 for each requested copy
  4. Mail your request to :

Note: Death certificates can be obtained by the decedent’s spouse, parent, child or sibling. The requester must provide either a VALID driver’s license, non-driver’s ID or passport and provide proof of relationship (spouse to provide a Marriage Certificate only; parent, child or sibling to provide a Birth Certificate only).  If you have any questions, please contact the Town Clerk’s office at (631) 957-3007.
